ScheduleDays vary, 10:30pm–8:30am (Full‑time, 40‑hour week).
Responsibilities- Collaborate with care team to ensure all residents access needed treatment, support, case management, and appointments, including transportation to external providers.
- Ensure all interactions represent the five principles of trauma‑informed care: safety, choice, trustworthiness, collaboration, and empowerment.
- Supervise medication windows to ensure the seven rights of medication use are followed.
- Actively participate in shift‑change meetings and complete associated documentation.
- Complete daily documentation in electronic medical record on resident interactions, medications dispensed when assigned, and other required documentation.
- Practice good financial stewardship of agency and contract resources.
- Meet identified key performance indicators and metrics for the position.
- Assist with and support on‑site cleaning, including skills training with residents to maintain personal and community spaces, laundry, and preparation for regular pest treatments.
- Provide immediate crisis intervention and resolution as needed.
- Always maintain resident and staff confidentiality.
- Complete assigned work related to maintaining and adhering to licensing responsibilities, such as Washington DSHS and DOH, as assigned by supervisor.
- Provide skills training and assistance to residents in ADLs and IADLs per their service plan and individual needs.
- Learn and execute skills and techniques promoted in the State of Washington mandated training, Fundamentals of Caregiving.
- Coordinate care, such as scheduling, attending, and/or arranging transportation for appointments and other case‑management needs.
- Maintain and document resident charts, activity logs, meal charts, etc.
- Provide input as part of resident’s care team on service plan and progress, and collaborate with other providers.
- Facilitate regular group and individual activities, encounters, and specialized resident events such as outings and holiday gatherings.
- Assist in admission and discharge planning per supervisor direction.
- Participate in fire drills, resident orientation, chores, and resident room maintenance during resident move‑in and move‑out.
- Monitor and manage facility milieu for visitors, television time, and quiet times.
- Attend all scheduled meetings and supervisions.
- Stay awake on all shifts.
- Perform other duties as assigned.
- Complete fingerprinting requirements within 14 days of hire.
- Obtain/maintain current Food Workers Card within 14 days of hire.
- Obtain/maintain valid First Aid/CPR card or certificate within 30 days of hire.
- Complete 75‑hour Long-Term Care Worker Basic Training plus mental health and substance use disorder specialty training within 120 days of hire.
- Obtain Home Care Aide certification within 200 days of hire.
- Complete 12 hours of continuing education annually.
- Prior experience with mental health population.
- Bachelor’s degree in social sciences or related field.
