Per-Diem Hospice Social Worker

Kline Galland Hospice is looking for a compassionate and experienced Hospice Social Worker to join our team on a Per Diem basis. This flexible role is ideal for professionals seeking meaningful work while maintaining a balanced schedule.

Responsibilities:

Conduct comprehensive psychosocial assessments for hospice patients.

Collaborate with the interdisciplinary team to develop individualized care plans.

Provide crisis intervention and therapeutic support to patients and families.

Coordinate community resources and referrals.

Deliver services in private homes, Adult Family Homes, Assisted Living, Independent Living, and Skilled Nursing Facilities.

Master’s Degree in Social Work (MSW), Marriage and Family Therapy (MFT), or Mental Health Counseling (MHC) from an accredited institution.

Minimum of 2 years of social work experience (1 year in Hospice or Home Health preferred).Strong verbal and written communication skills.

Proficiency in electronic charting and documentation.

Valid driver’s license, auto insurance, and reliable vehicle required.
