Community Health Worker

The Community Health Worker is a vital member of the clinic team, dedicated to improving health equity, supporting patients through education and navigation, and fostering strong community partnerships. The CHW acts as a bridge to resources and healthcare, supporting patients in overcoming barriers and navigating complex systems.

Required Qualifications:

  • Upon request:
    Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request. See policy for additional information.
  • 1 year Health Related, Community Outreach or Education Experience

Preferred Qualifications:

  • Bachelor's Degree Health Related, Community Outreach or Social Work Related Field Or equivalent educ/experience
  • Preference will be given to applicants with lived experience
