Overview

Job Title: Care Management Support Coordinator II

Contract Type / Duration: 4 months (possibility to extend or convert)

Location: Remote – open to candidates in DE, PA, NJ, MD

Work Hours: Monday–Friday, 8:00 AM – 5:00 PM (OT only with manager approval)

Position Overview

The Care Management Support Coordinator II provides outreach and support to health plan members. This role involves high-volume phone interactions with members, providers, and pharmacies to help close care gaps, connect members to resources, and ensure continuity of care. The position directly contributes to improving health outcomes and supporting members in navigating healthcare services.

Responsibilities
  • Conduct daily outreach calls to members, providers, and pharmacies.
  • Assist members with appointment scheduling, provider linkage, and access to community resources.
  • Support members with social determinants of health needs.
  • Identify members without a Primary Care Provider (PCP) and assist in obtaining one.
  • Screen members for eligibility in Care Coordination services.
  • Document assessments, outreach, and case notes in electronic systems.
Required Skills & Experience
  • Communication – Strong verbal and written communication skills; comfortable with 50+ daily calls.
  • Patient Assessment/Screening – 2+ years of experience in healthcare, managed care, or medical office settings.
  • Computer Literacy/EMR – 2+ years of experience with charting or electronic medical records.
Preferred Skills
  • Knowledge of medical terminology.
  • Experience supporting members with complex needs or high-risk conditions.
Education & Requirements
  • High School Diploma or equivalent required.
  • 2–3 years of experience in a managed care, healthcare, or medical office environment.
  • Must be eligible for W-2 employment (no C2C).
  • Must be legally authorized to work in the U.S. without current or future sponsorship.
