Manager of Population Health programs provides strategic direction and operational oversight for quality and value‑based programs managed through Pacific Medical Centers. Responsibilities include population health and value/risk‑based programs that foster, promote and depend on integration of services and collaboration across several acute, non‑acute, and health‑plan ministries.

• Bachelor's Degree in Healthcare Administration, Public Health or a related field.
• 2 years progressive health‑care leadership experience that includes healthcare services development, delivery, and operations.
• 2 years experience leading interdisciplinary care teams and planning complex geriatric and long‑term care.

• Master's Degree in Healthcare Administration, Public Health or a related field.
• 2 years experience developing and executing successful strategic business plans.
• 2 years experience supervising staff in a matrix‑structured organization.
• 2 years experience with project‑management methodologies and software such as CAP, Work‑Out and Lean.

$47.05 - $73.04 per hour (base pay). Additional compensation may be available for shift differentials, standby/on‑call, overtime, premiums, extra‑shift incentives, or bonus opportunities.
