Job Summary

Our behavioral health clinician is a core member of the collaborative care medicine (CoCM) team, which works together with our pediatricians and a consulting psychiatrist to address children’s mental health care needs and to facilitate improved functioning.

Salary Range: $75,000–$85,000 + full medical benefits, dental and vision

Responsibilities Direct Services
  • Support the behavioral and physical health care of patients on an assigned patient caseload. Closely coordinate care with the patient’s medical provider and, when appropriate, other mental health providers.
  • Screen and assess patients for common mental health disorders. Facilitate patient engagement and follow‑up care.
  • Provide parent education and coaching about common behavioral health problems, home behavioral management strategies, and available treatment options.
  • Systematically track treatment response and monitor patients (in person or by telehealth) for changes in clinical symptoms and treatment side effects or complications.
  • Support psychotropic medication management as prescribed by medical providers, focusing on treatment adherence monitoring, side effects, and effectiveness of treatment.
  • Provide brief behavioral interventions using evidence‑based techniques such as behavioral activation, RUBI, motivational interviewing, and other treatments as appropriate.
  • Facilitate treatment plan changes for patients who are not improving as expected in consultation with the medical provider and the psychiatric consultant and who may need more intensive or more specialized mental health care.
  • Develop and complete maintenance self‑management plan with patients who have achieved their treatment goals and are soon to be discharged from the registry.
Care Coordination
  • Closely coordinate care with the patient’s medical provider and, when appropriate, other mental health providers, schools.
  • Facilitate referrals for clinically indicated services outside of Hope Central (e.g., mental health specialty care).
Consultation
  • Participate in regularly scheduled (usually weekly) caseload consultation with the CoCM psychiatric consultant and communicate resulting treatment recommendations to the patient’s medical provider. Consultations will focus on patients new to the caseload and those who are not improving as expected under the current treatment plan. Case reviews may be conducted by telephone, video, or in person.
  • Support physician team in identification and vetting of patient facing psycho educational materials.
Chaplaincy
  • Serve as a counsellor and guide to the spiritual needs of Hope Central staff and patients.
  • Seek to motivate and initiate meaningful engagement of patients’ spiritual beliefs and practices in the management of their difficulties.
Administrative
  • Document patient progress and treatment recommendations in EHR and other required systems so as to be shared with medical providers, psychiatric consultant, and other treatment providers.
  • Generate end of month billing encounters.
  • Participate in program development and activities necessary for the sustainability of the position or new initiatives directly related to the position. This may include participation in grant planning, reporting, and execution of deliverables for direct funding.
  • Pursue continuing education activities necessary to maintain licensure.
  • Assist in education and support of staff around mental health and wellness subjects.
  • Participate in customary group meetings with clinical staff.
Qualifications Education and Licensure
  • Masters-level Licensed Independent Clinical Social Worker (LICSW) in Washington State
