Transfer Coordinator

Sevita, a leader in home and community-based health care, is seeking a Transfer Coordinator in Tukwila, WA. In this role, you'll coordinate safe, person-centered transitions of individuals between programs, homes, and care settings. Responsibilities include scheduling and documentation, collaborating with clinical and support teams, maintaining accurate records, and communicating with families and partners. Responsibilities

• Coordinate safe transfers between homes, programs, and care settings
• Schedule and confirm transfer logistics with staff, individuals, and families
• Maintain accurate, timely documentation in EHR and agency systems
• Communicate clearly with clinical and support teams to ensure continuity of care
• Monitor and report any safety or risk concerns during transitions
• Support individuals and families with information and resources about services
• Follow HIPAA and agency policies to protect confidentiality
• Participate in training and development to maintain required competencies
