Center Well Home Health is seeking a dedicated Home Health Aide to support patients in their homes under the supervision of an RN. You will follow the Plan of Care to provide direct care, assist with activities of daily living, and document patient progress to ensure continuity of care.
The role requires a CNA or CHHA program completion within the last 24 months, state certification, and a valid driver’s license with reliable transportation. Prior home health or CNA experience is preferred.
