Job Title
- Intake and Engagement Coordinator Schedule - M-F 6 am - 2 pm Location
- Onsite in Seattle

The salary range for this position is $31.25 to $33.25, with a midpoint of $32.25/hr
. New hires will typically receive between minimum and midpoint; however, offers may vary based on experience, internal equity, and market.

Job Summary
  • Supports ETS’ mission by facilitating onboarding and engagement for individuals seeking behavioral health services.
  • Plays a key role in the intake process, ensuring a smooth transition into care and supporting participant engagement during the early stages of treatment, including the first 90 days and other critical milestones while re-engaging participants as needed.
  • Collaborates with referral sources and internal teams to connect individuals to appropriate services—including case management, counseling, medical services, and other resources to promote sustained participation.
Responsibilities Intake & Onboarding
  • Serve as the first point of contact, providing a welcoming experience and an overview of available programs.
  • Conduct intake assessments, gather required information, and complete eligibility screenings.
  • Ensure all completed consent forms and documentation meet organizational and regulatory requirements.
  • Provides service information and connects individuals to appropriate programs based on identified needs.
Service Coordination & Navigation
  • Coordinates and schedules appointments across internal services, including case management, counseling, and medical care.
  • Facilitate internal and external referrals and assist participants in accessing services.
  • Act as a liaison between participants and providers to ensure prompt access, continuity of care, including warm hand-offs.
Engagement & Follow-up
  • Maintains ongoing contact to encourage engagement, especially during the early care and key treatment milestones.
  • Conduct outreach to individuals who miss appointments or services, assist with rescheduling, and documenting follow-up efforts.
  • Communicate with participants via phone, email, text, or in-person contact.
Barrier Identification & Support
  • Identify barriers to engagement (e.g., transportation, scheduling, childcare, financial constraints) and communicate findings to appropriate care team members.
  • Track common barriers to inform care coordination and service planning efforts.
Documentation, Tracking & Team Collaboration
  • Maintain accurate and timely records of participant interactions, intakes, referrals, and outreach efforts in the Electronic Health Record (EHR).
  • Track activity metrics (e.g., number of intakes, outreach attempts, appointment follow-up) in accordance with team processes.
  • Participate in team meetings and collaborate with internal and external partners to support coordinated care.
  • Ensure all documentation complies with organizational standards, HIPAA, and 42 CFR Part 2.
Community Engagement
  • Support outreach efforts and promote available services through community engagement activities and events.
  • Maintain knowledge of community resources to support appropriate referrals.

Requirements Qualifications Education  & Experience
