- Pharmacy Department has an outstanding opportunity for a Clinical Staff Pharmacist — ICU Float.

Full-Time / 40 hours per week Rotating/Variable Shift
POSITION HIGHLIGHTSAct as an integral member of HMC patient care services, providing pharmacotherapeutic recommendations to primary providers, nursing, and dietician staff to optimize patient care. Responsibilities include, but are not limited to direct patient care, drug distribution, research & education, and practice management as it pertains to the care of acute care patients.
PRIMARY JOB RESPONSIBILITIES- Drug therapy monitoring, including, but not limited to: ensuring appropriate drug therapy and nutrition, including indication, dose, drug interactions and drug allergies; evaluating pharmacotherapeutic regimens for effectiveness and adverse drug events, and intervene as needed; utilizing the medical record and other appropriate documentation systems to communicate with healthcare professionals and to document specific pharmacotherapeutic recommendations; providing drug information and education to medical teams, nursing and dietician staff;
providing patient counseling and education - Develop and maintain procedures to improve the efficiency and accuracy of medication distribution and administration
- Facilitate efficient medication delivery and appropriate scheduling of medications
- Provide primary preceptorship of PGY1 & PGY2 pharmacy residents and student pharmacists
- Provide education to medical/surgical residents, nursing staff and medical students
State of Washington Pharmacist License and one year of experience in hospital pharmacy practice. Months spent in the completion of an American Society of Health System Pharmacists accredited residency training program may substitute month-for-month for the required experience.
LEGAL REQUIREMENTState of Washington Pharmacist license.
