Optum's Senior Community Care team is seeking a Per Diem Nurse Practitioner or Physician Assistant to support patients in skilled nursing and transitional care settings through the Patient Connect Program
. The focus is on improving care coordination, reducing hospital readmissions, enhancing quality outcomes, and supporting patients and families during care transitions.

This is a highly autonomous role that involves conducting one-time comprehensive health assessments in collaboration with case managers and care teams.

• Per Diem position
• Minimum availability of 10 hours per week
• Flexible schedule including days, evenings, and weekends
• Maximum of 960 hours annually

• Conduct comprehensive patient assessments
• Collaborate with physicians, nursing staff, families, and case managers
• Support care coordination and transition planning
• Complete documentation, coding, and quality gap closure activities

• Active NP or PA license
• National certification (AANP, ANCC, NCCPA, etc.)
• DEA license (or ability to obtain)
• Valid driver's license and reliable transportation
• Ability to travel to skilled nursing facilities within the assigned territory
• Ability to work at least 10 hours per week

• 1+ year of post-graduate experience in Long-Term Care
• Experience with geriatrics, chronic illness management, or end-of-life discussions
• Strong documentation and EMR skills
• Ability to build relationships and adapt to change

Compensation for this specialty generally ranges from $90.00 - $155.00 per hour. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable.
