Department Description

Harborview Addictions Program (HAP) clinic and Harborview Mental Health Services (HMHS) clinics will coordinate medication assisted treatment along with collaborative, collocated, comprehensive mental health and substance use disorder treatment Services. HAP and HMHS clinics have a longstanding focus on dual diagnosis patients who are patients with chronic serious mental health issues and chemical and/or alcohol use/abuse issues. Many of these patients are also experiencing homelessness, have untreated mental and physical health conditions, and require legal and other case management services due to their opioid use disorder, highlighting the need for integration of MAT with comprehensive services.

Job Responsibilities
  • Coordinate patient access to Medication Assisted Treatment (MAT) and related healthcare services through intake, screening, referral management, and care coordination activities
  • Conduct outreach and collaborate with providers, care teams, hospitals, clinics, and community partners to identify eligible patients and facilitate access to treatment
  • Assess patient needs and connect individuals to medical, behavioral health, recovery support, social services, housing, transportation, insurance, and financial assistance resources
  • Serve as a primary point of contact for patients, families, providers, and community organizations regarding MAT services, eligibility, and available resources
  • Coordinate appointments, referrals, and communication among patients, providers, care teams, payers, and community partners to support continuity of care
  • Maintain patient registries, referral work queues, databases, and program records to ensure accurate documentation, reporting, and regulatory compliance
  • Monitor patient engagement and care coordination activities, identify barriers to care, and support strategies to improve treatment access and outcomes
  • Develop and maintain outreach workflows, referral processes, tracking systems, and operational procedures that support effective program delivery
  • Prepare reports, track program data, and participate in quality improvement initiatives to enhance program performance and patient services
  • Provide administrative and operational support, including responding to inquiries, maintaining records, supporting staff training, and participating in departmental meetings and initiatives
Requirements
  • High school graduation or equivalent AND two years of experience in the program specialty
  • Equivalent education and/or experience may substitute for minimum qualifications except when there are legal requirements, such as a license, certification, and/or registration.
