Health Coach

Responsibilities

• Deliver personalized, evidence-based coaching through virtual and in-person visits
• Support members in achieving sustainable behavior change, navigating health resources, and meeting wellness goals
• Conduct 30-minute individualized coaching sessions using motivational interviewing
• Co-create SMART goals and provide ongoing accountability
• Facilitate group coaching sessions and wellness workshops
• Administer screening tools and health measurements
• Maintain accurate, timely documentation in the EMR
• Collaborate with RNs, dietitians, medical providers, and behavioral health clinicians
• Support structured weight‑management programs
• Provide behavior‑change expertise to the multidisciplinary team
• Communicate via Slack and EMR to coordinate care
• Conduct proactive outreach to re‑engage members
• Manage scheduling, follow‑ups, and member panel activity
• Participate in wellness campaigns and client events
• Adhere to NBHWC Code of Ethics, HIPAA, PHI/PII protocols, and IT security standards
• Maintain a private, professional workspace for virtual visits
• Participate in call audits, QA reviews, and feedback cycles

• National Board-Certified Health & Wellness Coach (NBC-HWC) – Required
• 2+ years of professional health coaching experience
• Weight-management experience required
• Bilingual in Spanish – Strongly preferred
• Certified Diabetes Prevention Program (DPP) Lifestyle Coach – Strongly preferred
• Experience working with patients using GLP-1 medications – Preferred
• Must be able to work Pacific Time hours (8am–6pm PT)
• Must be comfortable completing up to 10 coaching visits per day (phone or video)
• Must submit a recorded assessment as part of the application process
• Experience delivering care via telehealth
• Salesforce and Google Workspace proficiency
• Strong understanding of coaching vs. education vs. behavioral health boundaries
• Proven track record of patient engagement and retention
• Adherence to NBHWC Code of Ethics, HIPAA, PHI/PII protocols, and IT security standards
• Ability to maintain a private, professional workspace for virtual visits
• Applications must include a recorded assessment demonstrating coaching style, communication skills, and bilingual fluency

Demonstrates expertise in delivering personalized health coaching, utilizing motivational interviewing techniques, and facilitating behavior change through structured programs. Proficient in managing virtual and in-person sessions while adhering to ethical standards and maintaining accurate documentation.
