Kidney Care Specialist

• Support patients with Chronic Kidney Disease and End Stage Renal Disease through early education, care navigation, and engagement
• Provide virtual care navigation regarding kidney disease progression, treatment options, home modalities, transplantation, and self-management
• Conduct scheduled and on-demand virtual or in-person patient interactions via video, phone, and electronic communication
• Assess patient needs, readiness, barriers, and preferences
• Collaborate with nephrologists, advanced practice providers, care teams, and internal partners to support coordinated care and referrals
• Build and maintain relationships with nephrologists, clinical managers, care coordinators, practice staff, and other stakeholders
• Provide referring practices with timely communication, education updates, and patient status
• Document patient interactions, education, and outcomes in Salesforce, EMR, or other approved systems
• Support referral workflows and patient follow-through across the kidney care continuum
• Track patient engagement, outcomes, and program performance using virtual tools, dashboards, and reports
• Participate in development and continuous improvement of virtual education materials and content
• Maintain current knowledge of kidney disease, treatment modalities, and Fresenius Kidney Care programs
• Identify barriers to care and connect patients with internal or external resources
• Support quality, safety, compliance, privacy, and documentation initiatives
• Participate in team meetings, training sessions, and performance reviews
• Support patients longitudinally and navigate next steps with the care team
• Travel to physician groups, healthcare locations, regional business units, corporate meetings, and on-site training

• High School diploma or G.E.D. required
• Minimum 1-3 years of experience in kidney care, chronic disease management, or related healthcare fields
• Valid driver’s license with reliable transportation and ability to travel within the assigned market
• Ability to travel extensively within the assigned market
• Willingness to work occasionally in the evening and weekends, as needed
• Proficiency in Microsoft Office applications
• Experience with CRM systems (e.g., Salesforce) and/or EMRs
• Comfort with digital platforms, virtual meeting tools, and data-driven workflows
• Strong communication skills with ability to explain complex clinical concepts clearly and in a patient-friendly manner
• Strong organizational and time-management skills with ability to manage a virtual caseload
• Ability to work autonomously but collaboratively on a team

Proficient in using Salesforce and EMR systems for documentation and tracking patient outcomes while maintaining strong communication and organizational skills.
