• Reduce barriers to medical care and to increase access to medication for HIV-positive patients DEPARTMENT DESCRIPTION The Max Clinic is an interdisciplinary primary medical care clinic that was developed in partnership between HMC
- Madison Clinic and Seattle-King County Public Health, STD Clinic. It is designed to engage patients who have extensive barriers to HIV care using incentives. Physically located at Harborview Medical Center in Seattle, the clinic serves over 200 patients, who access primary care on a walk-in basis. Max Clinic serves a patient population that is highly diverse, serving patients from various racial, ethnic, immigrant and LGBTQ communities.

PRIMARY

JOB RESPONSIBILITIES:

• Provide HIV/AIDS intake and case management to adult patients served in Madison Clinic.
• Conduct comprehensive assessments of psychosocial, financial, medical support and environmental needs
• Develop individualized case management service plans to assist the hospitalized and ambulatory care patient to obtain needed medical, social, financial, legal and home care services, housing, psychiatric and drug treatment and emotional support REQUIRED POSITION

QUALIFICATIONS:

A Master of Social Work Degree from a program accredited by the Council on Social Work EducationANDOne year of full-time social work experience in a health care setting or equivalent. Experience can include a practicum placement as part of a social work training program and/or employment experience.

LEGAL REQUIREMENTS:

Within 90 days of hire must hold one of the following credentials issued by the state of Washington:
• Agency Affiliated Counselor
• Licensed Social Work Associate
-Advanced
• Licensed Social Work Associate - Independent Clinical
• Licensed Advance Social Worker
