Overview
The Hospice Nurse Practitioner (NP) evaluates patients for hospice eligibility through comprehensive assessments, determining if they meet Medicare and regulatory criteria.
Responsibilities- Conduct face-to-face assessments, reviewing diagnoses and prognosis.
- Document findings accurately and consistently.
- Collaborate with the Medical Director, team, and other stakeholders.
- Support recertification processes for patients.
- Communicate care plans compassionately and ensure documentation compliance.
- Contribute to quality improvement initiatives.
- Master’s or Doctoral Nursing degree.
- Current NP license.
- National certification.
- DEA license.
- BLS certification.
2–3 years as an NP, preferably in hospice or palliative care.
Skills- Strong clinical assessment.
- Excellent documentation.
- Superior communication.
- Teamwork abilities.
- Proficiency in EMR.
Community-based. Requires local travel and in-person assessments.
Pay$54.52 to $65.66 per hour.
