Estimated Weekly Salary Range:  $3720 - $4410

Setting:  Dialysis clinics, nephrology practices, occasional telehealth

Flexibility:  Self-managed schedule; no nights, weekends, or on-call

Nurse Practitioner Qualifications

Minimum 1 year of Nurse Practitioner (NP) experience in healthcare settings

Current, unrestricted NP license in Washington State

DEA license or eligibility within 90 days

Current CPR/BLS certification

Valid driver’s license and insurance

Intermediate computer skills (MS Word, Excel, Outlook)

Strong clinical judgment and autonomous decision-making skills

Home office setup with high-speed internet and reliable transportation

Commitment to caring for patients with complex, chronic illnesses

Adaptability and a solution-focused attitude

Preferred:  Experience in Value-Based Care, Population Health, Medicare Advantage documentation, Health Risk Assessments, Cerner or similar EHR systems, and 5+ years in Family NP, Primary Care NP, Internal Medicine, Cardiology, Nephrology, or Endocrinology

Healthcare Responsibilities

Perform Comprehensive Health Evaluations (CHEs) for CKD and ESKD patients

Assess and manage comorbid conditions, addressing medical, social, and emotional needs

Lead care coordination to slow kidney disease progression and prevent hospital readmissions

Utilize prescriptive authority, medication management, and diagnostic interpretation in collaboration with nephrologists and interdisciplinary teams

Document patient care and assessments in EHR systems

Travel within assigned Seattle area for patient visits

Focus on improving patient outcomes and reducing hospitalizations
