RN - Case Management

We are seeking a Registered Nurse with experience in inpatient acute care and case management to join our healthcare team. This role involves providing care coordination for neuro and cardiac patients in a comprehensive stroke center.

• Care coordination for neuro and cardiac patients.
• Collaboration with interdisciplinary teams to enhance patient care.
• Participate in discharge planning and needs assessment.
• Utilize Epic for charting and documentation.
• Work with patients and families to develop a plan of care.

• Minimum of 2 years of experience in inpatient/acute care.
• BLS certification required.
• Active WA RN License.
• Experience with Epic charting system.
• Previous charge experience is preferred.
