In-Home Health - Nurse Practitioner or Physician Assistant; -Seattle​/King County, WA

Overview

Signify Health, part of CVS Health®, is seeking a Full‑Time Clinician (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations. The role involves traveling to patients’ homes within licensed states, conducting 45‑minute visits that include a medication and medical history review, a physical evaluation, and, when ordered, additional diagnostic tests such as diabetic eye exams or spirometry. Clinicians use an iPad and an intuitive clinical workflow, and they are not required to prescribe medication, order lab tests, or alter current treatment regimens.

• Provide in‑home health evaluations, connecting personally with patients and addressing their health questions.
• Perform medication and medical history reviews, physical assessments, and diagnostic tests as ordered.
• Travel locally and within licensed states as needed, and conduct virtual visits when required.
• Adhere to Signify Health’s clinical workflow and maintain accurate documentation.
• Ensure a safe and comfortable environment while evaluating patients in diverse home settings.

• Active, unrestricted licence(s) in the coverage area(s).
• Registered as an NP or PA with board certification (required).
• Ability to obtain and maintain multi‑state licences.

• Experience in in‑home or community health settings.
• Pre‑existing multi‑state licensure beyond the required area.

Pay is based on the number of completed in‑home evaluations and can vary by experience, education, and geography. Typical pay range: $95,738.00 – $ per annum.
