Job Overview

Signify Health, part of CVS Health, is seeking a Part-time Clinician (Nurse Practitioner or Physician Assistant) to provide independent In-Home Health Evaluations.

• The evaluation lasts about 45 minutes and includes medication and history review, physical assessment, and optional diagnostic tests (e.g., diabetic eye exams, spirometry).
• Clinicians use an iPad and intuitive workflow; no prescribing, ordering labs, or altering treatment regimens.
• Work primarily in members’ private residences; must be comfortable in varied home environments.

• Serve a national reach of millions of people, connecting them with appropriate care.
• Travel locally and within licensed states as needed and conduct virtual visits when required.
• Provide specified Diagnostic and Preventive Services products (e.g., spirometry, DEE).

• Active, unrestricted license(s) in the coverage area(s).
• Completion of 2,080 hours of post-graduate collaborative practice in an integrated clinical setting (and filing of attestation with the state of Minnesota).
• Board certification.
• Multi-state license(s) (preferred).

Pay Range: $41.85 - $90.13 per hour (base rate or full-time equivalent, dependent on experience, geography, and other factors)
