Overview

Transition Registered Nurse position at Fred Hutchinson Cancer Center. This is a 0.71 FTE (28.4 hours/week) role focused on transition and discharge teaching/planning for patients along the continuum of care, with coordination with home infusion/homecare agencies and skilled facilities.

Responsibilities
  • Assesses physical, emotional, social, and spiritual needs and evaluates patients' adaptation to health changes
  • Develops a plan of care based on patients' disease, symptoms, and response to treatment
  • Educates patients, families, and caregivers on disease processes, treatment, side effects and symptom management
  • Coordinates with the inter-disciplinary team to ensure continuity of care
  • Refers patients, families and caregivers to other clinical resources/professionals as appropriate (e.g., social work, home health care, clinical nutrition)
  • Implements, evaluates and modifies plan of care
  • Integrates research and evidence-based knowledge into clinical practice
  • Provide Transition Planning services at the UW Medical Center and Fred Hutch Clinic
  • Participate in assessment and evaluations of patient/family care throughout transitions
  • Coordinate patient care plans with home infusion/home care agencies and skilled facilities
  • Document assessments, interventions, and plans in the Medical Record after each patient encounter
  • Collaborate with a multi-disciplinary team to ensure continuity of patient/family care during transitions
  • Teach classes for patients and families in 1-on-1 and classroom settings
  • Serve as a liaison with insurance company case managers during treatment
  • Communicate effectively with other staff; act as Clinic ambassador to inpatient units, infusion companies, and referring offices
  • Support a patient- and family-centered philosophy
  • Participate in discharge planning with Transition Supervisor as needed
  • Discuss discharges at daily huddles and maintain appropriate workload balance
  • Follow transition report guidelines and act as a resource for the team
  • Be flexible in incorporating patient care outside own assignment to assist with needs
  • Demonstrate problem solving with other departments; adhere to Transition Program policies and standards
  • Participate in continuing education and stay current in area of expertise
  • Participate in professional organizations
QualificationsRequired:
  • Graduated from an accredited school of nursing
  • Current Washington State Registered Nurse (RN) license or current NLC Multistate License (MSL); must upgrade to an MSL with WA as primary state of residence within 60 days of hire (waiver possible in specific circumstances)
  • Oncology experience not required, with training and support provided
  • Current BLS AHA Health Provider Card (or equivalent) renewed every two years
  • Additional certification may be required by department
  • Critical thinking, clinical judgment, time management, and effective communication skills
Preferred:
  • 2 years nursing experience; 3 years in Bone Marrow Transplant, Immunotherapy or oncology preferred
  • Excellent teaching skills
  • Basic computer skills (Outlook, EPIC)
  • Experience in Health Insurance, Home health care, Discharge planning, Patient/family education, Ambulatory care
Compensation and Benefits

Hourly pay range: $47.64 to $88.19. Pay on a step-scale with possible shift differentials and other premiums (degree, preceptor, certification). Position is eligible for relocation assistance. Comprehensive benefits include medical/vision, dental, flexible spending accounts, life, disability, retirement, family life support, employee assistance, onsite clinic, tuition reimbursement, paid time off (vacation, sick leave, holidays), and parental leave. PTO prorated for part-time positions.
