Kline Galland Benaroya Services is seeking a dedicated and experienced Clinical Nurse Specialist to join our Home Health team. This is an in‑office role focused on supporting patients and clinicians through telephonic and virtual outreach, quality assurance, and interdisciplinary collaboration to maximize patient outcomes.
Key Responsibilities Clinical Support & Telehealth- Provide expert clinical guidance to nurses, therapists, and aides through phone consultations, interdisciplinary team (IDT) meetings, and in‑person interactions.
- Conduct telephonic and virtual patient assessments and follow‑up visits.
- Monitor patient progress, manage care plans, and document interventions.
- Review clinical documentation and patient data to identify improvement opportunities.
- Implement quality initiatives and ensure compliance with regulatory standards and best practices.
- Conduct outreach calls to patients and families to assess satisfaction and quality of care.
- Serve as a clinical liaison between providers, patients, and caregivers to align goals of care.
- Collaborate with intake teams to review referrals and ensure smooth patient onboarding.
- Develop and oversee initial and recertification Plans of Care (POCs).
- Ensure care plans are comprehensive, appropriate, and compliant.
- Identify patients who may benefit from telehealth/virtual services.
- Provide education and mentorship to clinical staff on best practices and new technologies.
- Lead or participate in interdisciplinary team meetings.
- Partner with leadership to promote high‑quality patient care.
- Analyze clinical data and prepare outcome reports.
- Perform in‑person visits on a PRN basis.
- Participate in Administrator On‑Call rotation.
- Support additional initiatives as assigned by the Clinical Leadership Team.
- Active RN or LPN in Washington State or RN or LPN Multistate license.
- Minimum 5 years of clinical experience, preferably in home health or a related setting.
- Strong clinical expertise in managing complex patient conditions.
- Exceptional communication and interpersonal skills.
- Experience with telehealth platforms and EMR systems (preferred).
- Strong analytical, problem‑solving, and organizational abilities.
- Demonstrated leadership and mentorship skills.
- Commitment to patient‑centered care and continuous quality improvement.
