A Clinical Nurse Coordinator provides clinical support to medical providers, managing care for their entire panel of patients. As the primary point of contact for complex outpatients, a Myeloma CNC delivers continuity of care through nursing assessments, triage, patient education, and care coordination. The clinical nurse coordinator is an integral part of a dedicated multidisciplinary team that collaborates closely to deliver the highest level of patient care.
This is a full‑time position at our South Lake Union location. The schedule for this position is Monday through Friday from 8:00 a.m. to 4:30 p.m.
Responsibilities- Assesses physical, emotional, social, and spiritual needs and evaluates patients’ adaptation to health changes
- Develops a plan of care based on the patient’s disease, symptoms, and response to treatment
- Educates patients, families, and caregivers on disease processes, treatment, side effects, and symptom management
- Coordinates with the interdisciplinary team to ensure continuity of care
- Refers patients, families, and caregivers to other clinical resources and professionals such as social work, home health care, and clinical nutrition
- Implements, evaluates, and modifies plan of care
- Integrates research and evidence‑based knowledge into clinical practice
- Educating patients and families: specific to rehabilitation services
- Telephone and email symptom triage
- Engage in meaningful patient follow‑up and care coordination
- Post‑procedure and medication side‑effect support
- New patient orientation to the service line
- Caregiver support
- Assist with in‑clinic procedures (setup, consent, time‑out, patient positioning)
- Assessment of needs for additional supportive care resources
- Prescription refills and addressing medication questions
- Result reviews
- Supportive care referrals
- FMLA paperwork, disability paperwork, ferry passes
- Coordination of outside clinical care and ensuring continuity with outside providers
- Collaboration with the research team for patients on trial
- Place physiatry return orders and supportive care referral orders as directed by the MD
- Coordinate responses to patient inquiries via MyChart and phone
- Coordinate lab tests and notify patients
- Coordinate with supportive care and medical care teams as needed across Fred Hutch sites
- Facilitate external referrals for ongoing rehabilitation needs
- Manage Epic Rehab Medicine Clinical Support InBasket inquiries from patients, staff, or other providers
- Attend regular team meetings with the provider and nursing teams
- Coordinate care with ancillary services
- Delegate tasks to appropriate team members
- Coordinate patient schedules with the clinic team
- Collaborate with the infusion RN and other members of the care team to ensure patient needs are met
