Job Description
UW Medical Center - Montlake Transplant has an outstanding opportunity for a Nurse Practitioner or Physician Assistant (ARNP/PA) to join our KIDNEY Transplant Team.
Work Schedule- 40 hours per week
- Day Shift
- Monday-Friday
The Advanced Practice Provider will provide specialized care for transplant patients and family assessment for Kidney care. This is a great opportunity to join our interdisciplinary collaborative team of outstanding providers.
PrimaryJob Responsibilities
- Conduct initial consults, as well as follow-up visits for patients.
- With keen attention to the specific needs of patients, deliver excellent care, with compassion, motivation, and sound medical knowledge.
- Coordinate and monitor patients’ status in conjunction with attending physicians, social workers, and other providers.
- Respond to patients’ healthcare needs, providing triage, urgent care and interventions as necessary/appropriate.
- Follow patients, including longitudinal care and follow-up, in conjunction with other providers.
- Nationally certified ARNP/PA-C with DEA.
- Current and valid Washington State Registered Nurse Licensure and ARNP Licensure or Physician Assistant License.
- Graduate Degree from an accredited Nurse Practitioner Program or graduate of an accredited PA program.
- Proven excellence working in an interdisciplinary collaborative team environment.
- Effective interpersonal skills, leadership skills, and clinical expertise in patient care.
- Experience in the provision of transplant services including initial assessment, diagnostic evaluation, and management of patients.
