The nurse case manager coordinates the care and service of selected patient populations through the acute care episode, across the continuum. Works collaboratively with inter‑disciplinary teams, both internal and external to the organization, to improve patient care through effective utilization and monitoring of healthcare resources and assumes a leadership role to achieve desired clinical, financial, and resource outcomes.
Required Qualifications- Bachelor's Degree:
Graduate of an accredited Bachelor of Nursing degree program or completion of an accredited Nursing program with a bachelor's degree or higher in a health care field, or - Applicants without a BSN degree must actively pursue and attain a BSN within three years of hire. Continued employment by Swedish beyond three years is contingent upon attaining the BSN degree.
- Upon hire:
Washington Registered Nurse License - 3 years Registered nursing experience in an acute care hospital.
- 1 year Hospital case management experience.
