Overview
The Advanced Practice Nurse (APN) Certified Nurse Practitioner (NP) in Cardiology provides a full range of services emphasizing quality, timeliness, and support of the VA mission. The NP delivers advanced clinical practice, consultation, and management.
Responsibilities- Provide and coordinate care for patients referred for structural heart and other advanced cardiology procedures
- Be an integral member of a growing cardiology program performing structural heart procedures, including transcatheter valve replacement/repair and left atrial appendage occlusion
- Evaluate patients, develop comprehensive plans, and implement and coordinate cardiac care in clinical and telehealth settings
- Exercise independent and autonomous decision making based on knowledge of nursing and cardiovascular diseases
- Coordinate care with referring physicians, cardiology and cardiac surgery teams, anesthesia, hospital medicine, and cardiac rehabilitation
- Collaborate with other APN providers to support clinical care and coordination for Cardiology patients needing general cardiology, heart failure, and electrophysiology management
- Practice to the full extent of education, training, and certification
- Operate within the clinical privileges outlined for this position
- Prescribe for a panel of patients or a program as indicated by the position
- Independently manage a panel of patients with acute and chronic conditions, emphasizing health promotion and disease prevention using evidence-based practice
- Develop and implement individualized plans to achieve optimal population health and disease management
- Demonstrate leadership and collaborate with the interdisciplinary team
- Order and interpret labs, tests, and diagnostic studies
- Prescribe pharmacologic and non-pharmacologic interventions
- Initiate referrals and consultations to specialty services
- Provide education to patients and their families
- Assess and adjust the plan of care as needed to achieve optimal patient outcomes
