$41.22– $61.54 per hour, exempt, payable bi-weekly. The salary scale for this position is between steps 0 and 19, depending on experience. Job Summary

The Triage Nurse is a key member of the multi-disciplinary healthcare team at Carolyn Downs Family Medical Center, supporting the clinic’s goals of delivering high-quality care and patient satisfaction. This role involves conducting patient assessments, triaging calls and walk-ins, providing direct patient care, coordinating with providers, and supporting the clinic workflow. The Triage Nurse also plays an important role in patient education, case management, and promoting continuity of care for patients, particularly those from underserved communities.

Job Responsibilities

Patient Triage & Assessment:

  • Triage patient calls and messages, assess needs, provide appropriate nursing advice, and schedule follow-up care or urgent evaluations as needed.
  • Triage walk-in patients and evaluate their medical needs based on history, vital signs, and physical assessment.
  • Independently see patients for services such as blood pressure checks, wound care, STI treatment, injection education, anticoagulation management, immunizations, and medication management.
  • Provide coverage and collaborate with specialty nurses in programs such as OB, diabetes, addiction medicine, population health, HIV/PrEP, and gender health.
Care Coordination & Communication
  • Communicate with external institutions (e.g., labs, schools, care providers) to coordinate patient care.
  • Support providers by relaying lab/imaging results, assisting with follow-ups, and facilitating care for complex or chronically ill patients.
  • Troubleshoot pharmacy and insurance-related issues; complete medication prior authorizations as needed.
  • Provide patient education on medications, side effects, dosing, and treatment plans.
  • Educate patients on reproductive health, infectious diseases, and chronic condition management.
Administrative Support & Documentation
  • Complete appropriate and accurate documentation of patient communication and care in EHR.
  • Participate in staff meetings, evaluations, and continuing education activities.

Qualifications

  • Experience working with medically underserved populations
  • Experience with EPIC EMR preferred
  • Ability to prioritize, multi-task, and work both independently and collaboratively
  • Demonstrated cultural competence and respect for diversity
