Overview

The Clinical Nurse Coordinator (CNC) provides clinical support to medical providers, delivering care management to their entire panel of patients. As the primary point of contact for complex outpatients, the CNC offers continuity of care through nursing assessments, triage, patient education, and care coordination. The CNC is part of a dedicated multidisciplinary team that collaborates closely to provide the highest level of care for patients.

The position is full‑time 1.0 FTE, located South Lake Union, Seattle, Washington. The work schedule is Monday‑Friday 8:00 am–4:30 pm.

• Assesses physical, emotional, social, and spiritual needs and evaluates patients’ adaptation to health changes.
• Develops a plan of care based on the patient’s disease, symptoms, and response to treatment.
• Educates patients, families, and caregivers on disease processes, treatment, side effects, and symptom management.
• Coordinates with the interdisciplinary team to ensure continuity of care.
• Refers patients, families, and caregivers to other clinical resources or professionals such as social work, home health care, and clinical nutrition.
• Implements, evaluates, and modifies plan of care.
• Integrates research and evidence‑based knowledge into clinical practice.

• Educating patients and families: specific to rehabilitation services.
• Telephone and email symptom triage.
• Engage in meaningful patient follow‑up and care coordination.
• Post‑procedure and medication side‑effect management.
• New patient orientation to the service line.
• Caregiver support.
• Assist with in‑clinic procedures (setup, consent, time‑out, and patient positioning).
• Assessment of needs for additional supportive‑care resources.
• Prescription refills and addressing medication questions.
• Review of results.

• Supportive‑care referrals.
• FMLA paperwork, disability paperwork, and ferry passes.
• Coordination of outside clinical care and continuity of care with outside providers.
• Collaboration with the research team for patients on trial.
• Place physiatry return orders and supportive‑care referral orders as directed by MD.
• Coordinate responses to patient inquiries via MyChart and phone.
• Coordinate lab tests and notify patients.
• Coordinate with supportive‑care and medical‑care teams as needed across Fred Hutch sites.
• Facilitate external referrals for ongoing rehabilitation needs.
• Manage Epic Rehab Medicine Clinical Support In‑Basket inquiries from patients, staff, or other providers.

• Attend regular team meetings with providers and nursing teams.
• Coordinate care with ancillary services.
• Delegate tasks to appropriate team members.
• Coordinate patient schedules with the clinic team.
• Collaborate with infusion RN and other team members to ensure patient needs are met.
• Real‑time management and escalation of issues in the clinic.

• Document all nursing care activities accurately and timely.
• Demonstrate knowledge and understanding of patient privacy rights.
• Actively participate in continuing education, departmental projects, and committees.

• Graduated from an accredited school of nursing.
• Current Washington State Registered Nurse (RN) license or NLC multistate license (MSL); must upgrade to an MSL with primary state of residence, WA, within 60 days of hire (waivable under specific circumstances).
• Applicants without prior oncology experience will receive additional training and support.
• Current BLS AHA Health Provider card (or equivalent).
• Additional certification may be required depending on the department.
• Critical thinking and cognitive skills for analysis, application, and judgment.
• Time‑management skills needed for independent organization and task management.
• Effective communication skills, including active listening, questioning, and clarifying.
• Ability to provide information clearly, avoiding jargon and checking for understanding.
