  • Takes accurate vital signs
  • Performs activities of daily living and assists patients to meet these activities
  • Participates in case conferences to discuss multidisciplinary team responsibilities and patient progress

  • Assists patient with ambulation as necessary
  • Supports patient safety and reduces healthcare errors

  • Participates in mandated activities as requested by Clinical Manager or Director of Patient Care Services
  • Documents clearly in the medical record in accordance with reimbursement guidelines and Agency policy
  • Identifies and communicates findings to the supervising RN
  • Works as part of the interdisciplinary team
Responsibilities
  • Understand regulations and standards applicable to Hospice
  • Demonstrate knowledge of the functions of a Hospice Agency
  • Demonstrate knowledge of the dying patient and pain control measures
  • Show genuine interest in and compassion for patients and families facing end-of-life issues
  • Understand hospice philosophy and issues of death and dying
  • Be flexible, organized, and able to function under stressful situations
  • Communicate effectively in English, both verbally and in writing
  • Excellent interpersonal, writing, and presentation skills
  • Knowledge of general modalities and scope of practice within the state of Agency operation
  • Self-directed with the ability to work in the field with minimal supervision
  • Valid driver’s license and reliable transportation; proof of automobile insurance (MVR checked)
  • Proof of current CPR certification
  • Able to pass criminal background check and national sex offender clearance
