Advanced Practice Nurse - Hematology ​/Oncology

Job Title

Independently manages a panel of cancer patients with acute and chronic health conditions with an emphasis on treatment, health promotion and disease prevention using evidence-based practice.

Works with physicians in the VAPSHCS Hematology Oncology Section to develop and implement individualized plans to achieve disease management and surveillance strategies.

Works collaboratively with Primary Care Team and other interdisciplinary team members towards patient specific goals.

Orders follow up and communicates diagnostic studies and results to patients and the hematology-oncology care team.

Prescribes non-oncologic pharmacologic and non-pharmacologic interventions.

Initiates referrals/consults to specialty services.

Provides education to patients and their families.

Assesses and adjusts the plan of care with hematology-oncology physicians as needed to achieve optimal patient outcomes.

Leads the site care team and organizes clinical care procedures, practice and work-flow within the clinic.

Pay:
Competitive salary, regular salary increases, potential for performance awards
