Join our Seattle, WA team! RN/LPN Day Center Supervisor,full-time, day schedule, leading patient-centered care for geriatric populations in a collaborative PACE program environment.

Requirements to Apply
  • Active RN or LPN license (state-specific)
  • Current BLS certification upon hire
  • Minimum 2 years of experience coordinating care for frail or elderly populations (within the last 5 years)
  • At least 1 year of full-time healthcare experience with interdisciplinary team collaboration
Job Overview

Our client is seeking a compassionate and experienced Day Center Supervisor to lead daily operations within a PACE (Program of All-Inclusive Care for the Elderly) setting. This role combines clinical oversight, care coordination, and team leadership to ensure high-quality, individualized care for frail and elderly participants. You will supervise staff, guide care planning, and support therapeutic programming while collaborating closely with interdisciplinary teams.

The ideal candidate is a hands-on leader, strong communicator, and patient advocate who is passionate about improving the quality of life and independence of aging populations.
