Signify Health, part of CVS Health, is seeking a Per diem Clinician (Nurse Practitioner or Physician Assistant) to provide in‑home health evaluations, visiting members in their homes and bridging gaps in care. Each visit lasts about 45 minutes with history review, physical exam, and optional tests.
You’ll travel locally within licensed states, conduct virtual visits as needed, and use an iPad workflow. Part‑time role with base pay and incentives, plus malpractice insurance and supplies.
