Home Health Registered Nurse, RN

• Provide skilled nursing care to patients in their homes while coordinating and managing their overall plan of care
• Perform comprehensive patient assessments and ongoing nursing evaluations
• Develop, implement, and update individualized care plans
• Provide skilled nursing services, including medication management, injections, wound care, and treatments
• Monitor patient progress and communicate changes in condition to physicians and care team members
• Educate patients and caregivers on diagnoses, medications, treatment plans, and disease management
• Coordinate care with physicians, therapists, social workers, and other healthcare professionals
• Complete accurate and timely EMR documentation
• Ensure compliance with Medicare, Medicaid, state, and agency regulations
• Participate in interdisciplinary team meetings and case conferences
• Promote patient safety, independence, and quality of life

• Active Washington State RN License
• Current BLS Certification
• Valid driver's license and insured vehicle
• Home Health or Hospice experience preferred, but not required
• Strong clinical, communication, and organizational skills

Demonstrates expertise in providing skilled nursing care, developing individualized care plans, and ensuring compliance with healthcare regulations. Strong ability to communicate effectively with patients and interdisciplinary teams while promoting patient safety and quality of life.
