2 days per week, ideally Monday & Tuesday (flexible)

8:00 a.m. – 5:00 p.m. onsite (30–60 min lunch)

• Patient Population:
  Adults only (18+)
• Daily Volume: ~18–20 patients per day
• Clinical Focus:
  • Preventive health services
  • Chronic illness management (HTN, diabetes, COPD, etc.)
  • Wellness and health maintenance
  • Integrated care within a behavioral health setting

• Provide comprehensive adult outpatient preventive care
• Manage chronic medical conditions
• Maintain a daily caseload of 18–20 patients
• Collaborate with an interdisciplinary team focused on whole-person wellness
• Document and coordinate care using Athena EMRMaintain high-quality, patient-centered clinical standards

• Active Washington ARNP license (or ability to obtain)
• Family Medicine background strongly preferred
• Experience in adult outpatient primary care and chronic disease management
• Prior work in community-based or integrated care settings preferred
• Strong communication skills and comfort with electronic health records
