Optum Home & Community Care seeks a Nurse Practitioner to perform in-home assessments across multiple states. You will manage acute illnesses, chronic conditions, and sub-acute rehab needs while collaborating with an interdisciplinary team.
Travel up to 85% is required; weekdays in home settings, with weekends at home when possible. Active NP certification (ANCC/AANP) and an unrestricted license are required, along with a valid DEA license and the ability to obtain a collaborative practice
