Telephonic RN Case Manager, Care - Washington and Colorado

About This Role

As a Telephonic RN Case Manager at United Health Group, you will serve as a primary point of contact for members with complex medical, behavioral health, functional, social, and long-term services and supports needs. This role involves conducting telephonic assessments, developing person-centered care plans, and coordinating community-based services.

• Pay: $60,200 - $107,400/year
• Location: Seattle, WA
• Schedule: Full Time, Day shift

• Conduct comprehensive telephonic assessments of members' needs
• Develop, implement, and reassess individualized, person-centered care plans
• Coordinate care across Medicare, Medicaid, and community-based programs
• Identify gaps in care and connect members with appropriate resources
• Facilitate safe transitions between care settings
• Assess changes in condition and apply clinical judgment
• Respond to and support resolution of member escalations
• Provide education on conditions, medications, and self-management
• Advocate for members and caregivers in care planning
• Document assessments and care plans accurately in electronic systems

• Active, unrestricted RN license or multistate RN license required

• 2+ years of professional RN experience
• Experience coordinating care for patients with complex needs
• Experience with Medicaid Long-Term Services and Supports or related programs

• Active RN license in Washington and Colorado required
