Base Pay: $23.75–$28.50 per hour, based on experience, qualifications, and work location.
Bonus & Incentive Potential:
Eligible employees can earn performance‑based bonuses and incentives, with the opportunity to bring total hourly compensation up to over $32.00 per hour.

As a Clinical Care Coordinator, you will oversee an assigned caseload of home care clients while serving as a clinical resource and leader for caregivers in the field. This position combines client assessments, supervisory visits, caregiver education, quality assurance, staffing collaboration, and direct client support to ensure exceptional client outcomes and continuity of care.
This is a full‑time position based on a 40‑hour work week. Approximately 75% of your time will be spent in the field conducting Start of Care visits, client assessments, supervisory visits, caregiver orientation, quality assurance visits, client introductions, and providing direct client support as needed.
