Transition RN - Oncology Care & Discharge

Fred Hutchinson Cancer Center seeks a Transition Registered Nurse (0.95 FTE, 38 hours/week) to provide transition and discharge planning for patients across the continuum of care. You will work with a multidisciplinary team, interfacing with home infusion/homecare agencies, and with the UW Medical Center and Fred Hutch Clinic.
