AAS – Case Manager I (Punjabi Speaking)
Asian Counseling and Referral Service (ACRS) is a social justice organization that promotes the health and well‑being of Asians, Pacific Islanders, and other underserved communities by providing and advocating for responsive community‑based services.

- Develop and implement person‑centered, holistic care plans including comprehensive in‑home functional assessments.
- Coordinate community resources, manage referrals, and ensure services meet client needs.
- Authorize services and manage Medicaid–funded services through Provider One.
- Advocate for clients in areas such as citizenship, housing, food assistance, financial assistance, legal advocacy, and translation/interpretation.
- Facilitate transitions to nursing homes, assisted living facilities, and adult family homes, coordinating placement and continuity of care.
- Act as a mandatory Adult Protective Services reporter, completing timely reports of suicidal ideation or other mental/behavioral issues.
- Assist clients with home modifications, provide short‑term crisis intervention, and determine appropriate care settings.
- Maintain timely documentation, participate in quality assurance, contract compliance, and program performance initiatives.
- Attend agency, program, and team meetings, consultations, and trainings.
- Education and Experience
- Master’s degree in social work, behavioral or health sciences with bilingual or bicultural skills.
- Bachelor’s degree in related field with bilingual or bicultural skills.
- Associate of Arts/Science with at least one year of paid on‑the‑job social services and bilingual or bicultural skills.
- High school diploma with at least three years of paid on‑the‑job social services and bilingual or bicultural skills.
- Completion of a Case Manager training program, including State‑sponsored CORE Training, is beneficial.

- Provide respectful quality care responsive to cultural beliefs and practices.
- Fluency in an Asian and Native Hawaiian/Pacific Islander language preferred; multilingual skills highly valued.
- Knowledge and Skills
- Proficient customer service skills with empathy, compassion, and patience.
- Strong time‑management, problem‑solving, and critical thinking.
- Conflict resolution skills to mediate between clients and caregivers.
- Knowledge of aging processes, community service delivery systems, and Title XIX Case Management programs.
- Understanding of common adult illnesses and their impact on daily tasks.
- Sound judgment regarding safe environments during client assessments.
- Technical Skills
- Competency in Microsoft Office (Word, Excel, Outlook), Teams, Zoom, and web‑based applications.
- Efficient internet research skills.
- Additional Requirements
- Reliable transportation and valid driver’s license; vehicle insurance must be current if using a personal or agency vehicle.
- Compliance with all ACRS policies as outlined in the Employee Handbook.
- Pay range: $58,900–$63,700 annually (commensurate with experience).
