Job Description

Asian Counseling and Referral Service (ACRS) is a social justice organization that promotes the health and well‑being of Asians, Pacific Islanders, and other underserved communities in King County and beyond. ACRS seeks a full‑time Care Coordinator I (Vietnamese speaking) in the Aging and Adult Services department to support highly vulnerable adults who are elderly and/or living with disabilities with complex health and social needs.

Benefits
  • Pay range is between $58,900 to $63,700 annually. Can increase based on experience.
  • ACRS pays 100% for employee only HMO/Virtual Plus medical benefits. Additional options are covered anywhere from 75% to 85%.
  • Dental and Vision insurance are available at a very minimal cost.
  • ACRS pays 100% of your life, AD & D, and long‑term disability insurance on or coinciding with your date of hire.
  • Employer paid ORCA card and vanpool program. Free on site parking available at Main Office.
  • Paid Time Off:
    • 8 hours of vacation accrued monthly from day 1, available to use after 6 months of employment.
    • 6 hours of sick leave, accrued monthly from day 1, available to use once accrued, no waiting period.
    • 32 hours (max) of wellness leave per year, available to use upon hire, no waiting period. Wellness leave hour amount is subject to proration based on the calendar month of hiring.

Responsibilities
  • Conduct in‑person visits monthly at client’s home, healthcare facility, social service agency, and/or community settings by client preference to ensure continuity of care and coordination and to reduce gaps in services. Administer assessments quarterly, develop Health Action Plans with measurable objectives, and provide health coaching to enhance clients’ self‑management skills and engagement with care providers. Identify resources, manage referrals, assist with maintaining eligibility for essential services, and foster communication between healthcare providers and networks.

    Support transitional care after hospital discharges and recognize the role of family, caregivers, and decision‑makers in navigating healthcare and social services.
  • Ensure accurate and timely documentation and billing procedures in reporting activities and services provided.
  • Outreach to potential clients who are referred to the programs by explaining the benefits of the program and importance of being actively engaged in their own health and medical needs. Assist potential clients and their families in completing the required enrollment documents.
