Social Work Case Manager

Swedish Health Services is seeking a full‑time Social Work Case Manager to provide a full range of social work services including psychosocial assessment, treatment planning, therapeutic interventions, discharge planning, crisis intervention, and resource referral. The role involves partnering with the patient and their support system, as well as interdisciplinary teams, to improve patient care through the effective utilization and monitoring of health care resources and to facilitate the achievement of patient goals and desired clinical, financial, and resource outcomes.

• Provide psychosocial assessment and treatment planning.
• Deliver therapeutic interventions and crisis intervention.
• Coordinate discharge planning and resource referral.
• Collaborate with patients, families, and interdisciplinary teams.
• Monitor and utilize health care resources to support patient goals.

• Master's degree in Social Work.
• 1 year experience as a Social Worker or similar.
• Washington State Social Worker Independent Clinical License.
• 1 year of experience in acute medical or mental health care in inpatient or outpatient settings.
