Overview

Catholic Housing Services believes everyone deserves the right to a safe, affordable place to call home. Inspired by this vision, CHS develops, owns and manages affordable housing programs for low-income families and individuals; currently including 2,100 units of housing in 50 different buildings across Western Washington. We provide supportive services for people who have experienced homelessness, low-income seniors, farm worker families, and those who have special physical and mental needs.

Catholic Community Services and Catholic Housing Services work together in partnership to provide permanent supportive housing to individuals and families who have experienced homelessness in King County. We believe that everyone deserves a safe, affordable place to call home. We understand that people experiencing homelessness come to us with complex histories, and that it takes a highly collaborative, creative team to help end the cycle of homelessness permanently.

The Josephinum is a 221‑unit historic building in the Belltown neighborhood of Downtown Seattle. Josephinum provides studio and 1 bedroom apartment homes and supportive services to formerly homeless persons. The street level of the building is shared with Christ our Hope Catholic Church. There are several other social service programs occupying space in the building as well.

This position offers a compensation range of $27.27 – $30.34 per hour (DOE)

Full‑time, 40‑hour position include a competitive benefits package:

Responsibilities

Case Management:

  • Complete new resident intake and case management plans. Assess resident needs and required levels of services, identifying community resources to meet those needs, facilitate residents’ access to services and advocate for residents on a continuing basis.
  • Develop a specific Housing Service Plan, which will address the identified needs as well as the areas which have been concerns for the resident in maintaining previous housing.
  • Initiate ongoing engagement with residents through intensive outreach, activities, groups, or other events that promote a healthy community.
  • Provide case management to residents. Assess resident needs and with the resident create a plan to meet those needs. This may include finding community resources, facilitating resident access to services, and advocating residents on a persistent basis to ensure consistent delivery of service. Case management services to residents include, but are not limited to, accessing financial and health benefits, primary health care, mental health treatment, chemical dependency counseling, chore service, protective payee, transportation, food stamps and legal services.
  • Create and maintain documentation of services and service plans in resident files and online HMIS reporting. This will include resident contracts, referrals, and interactions in case notes.
  • Assist residents with tracking of appointments and transportation; as well as accompanying residents when appropriate to appointments.
  • Make home visits to resident apartments to ensure resident welfare and to confirm the apartment is in a safe state and in compliance with all city and federal housing quality standards.
  • Complete monthly and quarterly services and demographic reporting. Maintain accurate client files and compliance with funder data collection requirements.
  • Facilitate group activities for residents on a regular schedule and at a minimum of twice a month.

Supportive Services/Resident Relations:

  • Work with the entire team to keep residents in housing through individual conversations with residents, Care Conferences, advocating for additional services for residents and other creative strategies to keep residents in housing.
