Case Manager - Diversion Float

Community House Mental Health Agency seeks a Diversion Float Case Manager to support outpatient behavioral health services and community support across all three Diversion teams. The role coordinates care, ensures documentation, provides client engagement, and supports team-based service delivery.

• Serve as a Float Case Manager covering program needs when short‑staffed, advocating for and coordinating client needs and services; ensuring Service Plans, Crisis Plans, and other documentation are completed on time and kept up‑to‑date.
• Coordinate with internal and external departments on resident referrals, ensuring timely intake into the program and review of individualized plans.
• Collaborate with various staff (OCRP, FHARPS, FPATH) to identify client barriers, develop plans, provide group facilitation, interpreter services, and administer urinalyses for OCRP.
• Outreach clients in their homes or community stays to assess living situation, stability, and provide interventions when necessary.
• Ensure all interactions represent the five principles of trauma‑informed care.
• Coordinate medication access, ensuring the seven rights of medication use are followed.
• Complete daily documentation in the electronic medical record for billable and non‑billable activity, medication dispensation, and other required documentation.
• Practice good financial stewardship, monitor petty cash, and ensure proper use of agency resources.
• Meet key performance indicators and metrics for the position.
• Participate in discharge planning, written summaries of services, and crisis intervention as needed.
• Assist with drop‑in center staff rotation and maintain client and staff confidentiality.
• Collaborate with the care team to ensure clients access needed services, support, case management, and appointments, including transportation to external providers.
• Provide and receive input as part of resident’s care team on service planning and progress, and collaborate with other providers such as primary care, psychiatric prescribers, and housing staff.
• Facilitate regular group and individual activities, and specialize client events.
• Perform other duties as assigned.

• Bachelor’s degree in social science or related field, or 2+ years of relevant experience serving adults with severe and persistent mental illness and/or in diversion services in lieu of a degree.
• For the FHARPS program: credentialing as a Certified Peer Counselor is acceptable and preferred.
• Apply for Agency Affiliated Counselor credential with Washington Department of Health within 30 days of hire and maintain registration and/or licensure.
• Successfully pass a criminal background check before hire date.
• Obtain/maintain a valid First Aid/CPR card or certificate within 30 days of hire.
• Hold a valid license for 1 year with clean driving record to use agency vehicles.
• Have legible handwriting and ability to perform simple mathematics.
• Able to navigate and comprehend electronic medical record charts.
• Able to work independently and within a team.
• Able to communicate information and ideas clearly and exchange accurate information.
