Case Manager Social Worker

Responsibilities

• Conduct comprehensive assessments to identify client needs, strengths, and goals.
• Develop and monitor individualized care and service plans.

• Advocate for clients to access benefits, resources, and support services.
• Provide crisis intervention and safety planning as needed.
• Maintain accurate, timely documentation in electronic records systems.
• Monitor client progress and adjust plans based on outcomes.
• Educate individuals and families on available services and supports.
• Participate in interdisciplinary team meetings and care reviews.
• Ensure compliance with agency, state, and regulatory standards.
