Health Care Benaroya Services at Kline Galland is looking to immediately hire a Full-Time Social Worker to join our amazing Hospice and Palliative Care team. This is a meaningful opportunity to make a difference by supporting patients and families during some of life’s most important moments.
Service area:
King County
- Conduct comprehensive psychosocial assessments
- Develop and implement plans of care in collaboration with the interdisciplinary team
- Provide crisis intervention and therapeutic support
- Coordinate resource referrals and community services
- Support patients and families navigating end-of-life care
- Provide care across multiple settings, including:
- Private residences
- Adult Family Homes
- Independent & Assisted Living communities
- Skilled Nursing Facilities
- Master’s Degree MSW from an accredited program
- Minimum 2 years of social work experience
- 1 year in Hospice and/or Home Health preferred
- Strong verbal and written communication skills
- Proficiency in computer charting and documentation systems
- Valid driver’s license, auto insurance, and reliable transportation
