Field RN Home Health Visits in Sebastian / Barefoot Bay

The RN will work to provide the highest quality of care to patients utilizing the Plan of Care provided to ensure patients' needs are met in relation to safety, well‑being and satisfaction. Well Sky/Kinnser, Oasis, and Home Health experience is a plus.

• RN will be supervised by an DON.
• Provides safe and effective care for the client based on individualized Plan of Care.
• Provides prompt communication of changes in patient medical status/emergencies to the Team resulting in effective management of cases.
• Provides safe and effective nursing for the patient, based on the nursing scope of practice and individualized Plan of Care in conjunction with the Nurse Supervisor/Manager of Clinical Practice. May include, but is not limited to, medication administration, wound care, infusion, transferring & positioning patient, catheter care, effective communication with the client, family, and Team Case Manager.
• Demonstrate empathy, caring, and self‑confidence; pay attention to details resulting in thorough follow‑through; anticipate problems in advance and know whether to diffuse or elevate; prioritize and demonstrate a sense of urgency.

• Current Nursing license, state of Florida.
• Possess wound care skills.
• Excellent communication skills, team player, interpersonal skills, good collaboration skills, customer service skills; ability to use the electronic health record for documentation and communication regarding client care.
• One year of Home Health experience preferred.
