LPN Home Health

Advent Health is seeking a Licensed Practical Nurse (LPN) for home health care. The role includes patient education, medication administration, wound dressing, and administrative duties.

• Provide direct patient care under RN supervision, including medication, wound dressing, ostomy care, and other clinical tasks.
• Administer treatments and medications as ordered by providers, ensuring safety and documentation.
• Perform diagnostic assays such as EKG, spirometry, audiometry, pulse oximetry, titmus, and forensic sample collection.
• Assess and monitor patients, documenting vitals and reviewing signs of abuse or neglect.
• Maintain clinical records in accordance with established standards and time‑frames.
• Participate in staff development, quality improvement, and performance improvement programs.
• Assist in call rotation and provide weekend/after‑hours coverage.
• Communicate changes to RN, physician, and the home health care team.

• Licensed Practical Nurse (LPN) license.
• Basic Life Support (BLS) certification; IV certification, driver’s license preferred.
• High school graduate or equivalent; technical/vocational school graduate in licensed practical nursing.
• 1+ year of medical/surgical experience preferred.
• Proficient in English; other languages preferred.
• Knowledge of practical nursing theory, modalities, treatments, and venipuncture skills.
• Basic computer skills, including MS Office; experience with point‑of‑care software preferred.
• Strong communication, multitasking, organizational, and critical‑thinking skills.
• Background check meets organizational guidelines.
• Ability to make independent emergency judgments and decisions.
• Flexible and able to work under busy and stressful conditions.

Full‑time, Monday‑Friday 8 am – 5 pm with weekend on‑call rotation. 12903 N Telecom Pkwy, Temple Terrace, FL 33637. Covers Hillsborough, Pasco, Pinellas, Polk, Sebring/Highlands/Hardee counties.
