Home Health Licensed Practical Nurse-PRN

As a Center Well Home Health nurse, reporting to the Clinical Manager, you will provide personalized, one‑on‑one care that helps patients regain independence in the comfort of their homes. Working with a dedicated team of physicians and clinicians, you’ll manage care plans that support recovery and help patients get back to the life they love.

• Provide skilled nursing care to patients, working onsite in their homes, under the direction of an RN.
• Implement individualized care plans in collaboration with the patient, family, and healthcare team.
• Educate patients and their families on disease management, treatment options, and self‑care techniques.
• Maintain accurate records of patient care, including documentation of patient visit, interventions, and outcomes.
• Maintain contact with patients, physicians, clinical manager(s), and other members of the healthcare team promptly regarding patient or family needs and status changes.
• Participate in care coordination activities and discharge planning.

• Current nursing license in the practicing state.
• Valid driver’s license, auto insurance and reliable transportation.
• Current CPR certification.
• Two years experience as an LPN/LVN in a clinical setting.

• Nursing experience in a Home Health or Hospice setting.

Pay per visit/unit: $33.00 - $47.00.

• TB Statement:
  This role is patient‑facing and part of Humana’s Tuberculosis screening program. If selected, you will be required to be screened for TB.
• Driving Statement:
  Requires a valid state driver’s license, personal vehicle liability insurance, and reliable transportation. The vehicle insurance must meet the state’s minimum required limits.

  40 hours per week (full time). Compensation range: $53,000 – $73,000 per year, depending on geographic location, experience, and other factors.
