Position: Per Diem RN-Sebring

Job Overview

The Per Diem RN is responsible for assessing and identifying patient and family needs, utilizing the nursing process, coordinating the Plan of Care with the Interdisciplinary Group (IDG), and providing palliative and supportive care to the patient/family unit. This role requires flexible scheduling to meet the needs of Hospice House patients and the ability to manage the emotional stress of caring for terminally ill patients and their families.

Responsibilities
  • Assess and identify patient/family needs, utilizing the nursing process.
  • Coordinate the Plan of Care with the IDG to provide comprehensive palliative and supportive care.
  • Complete clinical admissions, including explaining services, addressing questions, identifying objections, and educating families on admission requirements.
  • Obtain informed consents and initial assessment documentation in accordance with regulations and company policy.
  • Provide information to physicians, initiate Plan of Care, and engage the physician team in ongoing care planning.
  • Initiate skilled nursing interventions to prevent complications, alleviate symptoms, and maximize physical and emotional comfort.
  • Obtain and document physician orders and complete documentation per company policy.
  • Act as the company representative at assigned facilities, facilitate referrals to all service lines, and work closely with referring hospitals, physicians, and case managers.
  • Communicate promptly with Admissions and facilitate timely follow‑up, using SBAR for clinical communications.
  • Arrange safe discharge for referred patients, ensuring timely completion of the discharge process while meeting productivity expectations.
  • Provide care for geriatric inpatient (GIP) patients in hospitals, including daily visits, professional management, documentation, and efficient discharge.
  • Supervise care provided by other nursing team members (LPNs, Hospice Aides, and contract nurses).
  • Attend patient deaths, prepare bodies for transport, oversee disposal of medications, and provide bereavement interventions to family and caregivers.
  • Present concise oral and written reports to the IDG, respecting multidisciplinary input.
  • During emergencies such as hurricanes, report to alternate location as directed by the company to ensure continuity of services.
  • Perform other duties as assigned.
Qualifications
  • Valid RN license in the state where duties will be performed.
  • Active Basic Life Support (BLS) certification for healthcare professionals from the American Heart Association or Red Cross.
  • Minimum of one year of nursing experience; hospice or hospital experience preferred.
  • Previous experience with an EMR/EHR system.
  • Mobile driver status with a valid driver’s license, automobile insurance per company policy, and reliable transportation to meet the visit schedule.
  • Ability to use equipment with visual and auditory mechanisms.
  • Effective verbal and written communication in English.
  • Capacity to manage emotional stress associated with caring for terminally ill patients and families.
  • Physical ability to access patient rooms, hospital, skilled nursing facility, and any home setting, including navigating stairs and narrow spaces.
  • Capability to perform essential functions such as lifting patients or equipment, bending, pushing/pulling, kneeling, and other tasks with or without reasonable accommodation.
  • For PACE locations, completion of Alzheimer’s Disease and Related Dementias training through the Florida Department of Elder Affairs.
  • Cross training to hospice house or field positions when census requires flexible staffing.
Physical Demands
  • Stand, sit, reach with hands/arms, lift, bend, balance, push, pull, kneel, and occasionally stoop or crouch.
  • Capable of walking, climbing stairs, and performing tasks requiring physical exertion such as pull, push, and kneel.
  • Use of sensory faculties (see, hear, talk) is required.
Compensation & Administration

Pay rate: $45.00 per hour. The position requires consent to drug and/or alcohol testing after a conditional offer of employment, as well as ongoing compliance with the Drug‑Free Workplace Policy. All employees performing services for Florida affiliates are submitted to the Florida Care Provider Background Screening Clearinghouse to verify eligibility after a conditional offer of employment and for ongoing eligibility.
