Overview

This position is based in Sebring, FL and requires daily travel within Highlands and/or Hardee County. It is full‑time, Monday through Friday, 8 a.m. – 5 p.m. with participation in the on‑call rotation.

Salary Range

$80,000–$85,000 annually, with additional bonus opportunities. Final compensation may vary based on location, job‑related knowledge, skills, and experience.

Role of the RN Case Manager

As a Hospice Case Manager, you will have a special opportunity to provide, coordinate, and direct the provision of hospice nursing and palliative care to terminally ill patients in their home and wherever they call home. Our hospice nurses will closely collaborate with the attending physician, patients, and families, and other members of the patient care team to provide and maintain continuity of patient care to achieve excellent pain and symptom management and high-quality end‑of‑life care for the patient.

We service an adult population, providing support to patients and families.

Hospice RN Case Managers work Monday – Friday, 8 a.m. – 5 p.m. with a potential weekday on‑call and holiday coverage schedule. Case manage 15–18 patients, attend Interdisciplinary Team Meetings, contribute to team huddle calls, and receive updates on your patients daily.

  • Assign Aide Visits
  • Coordinate care with Social Workers, Chaplains, Volunteers, and Bereavement team
  • Develop and nurture relationships with facility staff and leaders throughout the community

Orientation, Onboarding & Training

You will then continue your onboarding working directly with nurses in our program. On average our orientation for an RN is 3–4 weeks, however, is individualized according to your needs and experience.

Responsibilities
  • Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
  • Make referrals to other hospice care disciplines as needed or document rationale for not doing so.
  • Develop, prepare, and maintain individualized patient care progress records with accuracy, timeliness and according to care center policies.
